Health Advocacy & Case Management

The health care system is not designed to take care of our older medically fragile patients.  Typically an office visit with a medical provider is about 10 minutes long.  Unfortunately that does not allow for a holisitic evaluation of our seniors and is a recipe for misinformation, inaccurate diagnoses and prescribing of to many medications.  This can lead to health problems that are not well managed, increased ER visits and hospitalizations and a decreased quality of life. Amanda understands this problem only to well, as a primary care provider herself in both the clinic and community setting she recognizes not only the challenges that affect the medical provider but the patients and families as well. Amanda offers a health advocacy and case management service to help families and patients navigate the health care system and truly understand their medical problems, medications and treatment options available.  Amanda has a truly holistic approach in caring for patients and ability to problem solve and offer solutions to these problems. This service is ideal for the elderly medically fragile patient whom may not have close family support geographically and needs an advocate to help direct their medical care.

Services Offered:

  • Thorough evaluation of patients medical status, medications and goals of care with ability to either accompany patients to their medical providers visits or provide phone consultation as needed
  • Co-ordination of patients care with family, medical providers and care facilities as well as oversight of residents care at their facility
  • This service can be provided on a case by case situation or there can be an ongoing monthly case management/health advocacy service


  • Rates are negotiable depending on the service requested and Amanda welcomes questions about the service she can provide and will try to work within the budget of the individual client.
